Amorphous powder, white or white with a light yellow tint, odorless.
Composition
1 g of the drug contains active ingredients:
- gentamicin sulfate – 0.024 g,
- L-tryptophan – 0.014 g,
- zinc sulfate heptahydrate – 0.010 g,
- methoxane – 0.952 g
Pharmacological properties
Kvadro is a combined antimicrobial drug of prolonged action for local use, containing an aminoglycoside of gentamicin sulfate, an organosilicon sorbent – polymethylsiloxane (methoxane) and a coordination compound of zinc with tryptophan.
Gengamycin is a broad-spectrum antibiotic with a bactericidal effect, active against pathogenic gram-positive microorganisms (Staphylococcus aureus, Streptococcus, Mycobacterium spp.) And gram-negative microorganisms (Pseudomonas aeruginosa, Aerobacter aeruginosa, Escherichia spp., Protelebsella spp., Protele spp. Spp. , Shigella spp., Salmonella spp., Citrobacter spp., Providenta spp., Yersinia spp.). The mechanism of action of gentamicin is irreversible connection with the ribosomal RNA (rRNA) subunit of bacteria. This linkage results in mRNA misreading, resulting in the production of non-functional or toxic proteins that cannot support bacteria.
Polymethylsiloxane (methoxane) provides sorption and detoxification properties and, together with zinc tryptophan, potentiates the antibacterial effects of the antibiotic and maintains the antimicrobial concentration of the antibiotic in the wound at a therapeutic level for 8 days.
Zinc has the property of activating repair processes, which is due to its participation in the synthesis of proteins, in particular collagen. When applied topically, zinc prevents the development of infection and stimulates the processes of epithelialization of wounds.
Tryptophan has a regulatory effect on the production of immunoglobulins IgA, IgG. IgM and IgE. Promotes the normalization of humoral immunity indicators in postoperative purulent-septic complications, increasing the content of CD19 + lymphocytes and stimulating the production of lg A and lg G. stimulates the functional activity of neutrophils and blood monocytes, activates phagocytosis. Stimulates the processes of tissue regeneration, accelerates wound healing, activates the functions of connective tissue cells, endotliocytes, macrophages and leukocytes in the focus of damage.
Application therapy with Quadro reduces the number of microorganisms in the wound for 2-3 days to a minimum level (10 2 -10 3 per 1 cm 2 ), accelerates the transition of the hydration phase to the dehydration phase, and prevents the development of possible purulent-septic complications. Due to its sorption properties, the drug absorbs toxic waste products of microorganisms, low molecular weight toxic metabolites of tissues (pyruvic and lactic acids, peroxide compounds, amino acid oxidation products, polypeptides, lipids, etc.), fibrin dehydration products. The mechanism of action of the drug Kvadro is associated not only with the inhibition of the synthesis of proteins of microorganisms, but also with a violation of the lipid layer of the cell wall due to the formation of complexes.
The drug Kvadro reduces the manifestations of local and general intoxication due to antibacterial properties, drainage and anti-edema action, normalization of microcirculation, gas exchange, pH of the environment in the wound. This helps to cleanse wounds from exudation products and tissue decay, eliminate local inflammatory reactions, stimulate the growth of granulation tissue, and improve the healing process. The drug Kvadro also cleans wounds with burns, reduces inflammatory reactions during their treatment, reduces the area of deep burns, preventing tissue destruction, accelerates the formation of a granulation cover in deep burns and epithelialization in superficial burns.
Application
Treatment of dogs, cats, horses and pigs (boars after castration) for traumatic and surgical wounds, burns, trophic ulcers, dermatitis caused by microorganisms sensitive to the active ingredients of the drug.
Dosage
Externally, locally. The drug is used throughout the treatment of wounds until they are completely healed. In the initial stage (1-5 days), the dressings are changed 1-2 times a day, after reducing the inflammatory processes, cleansing the wound from purulent-necrotic masses, the appearance of granulation and epithelialization of the wound – once a day for 1-2 days. The drug is applied to the surface of the wound through a dropper plug (hole in the plug), removing the cover, after thorough surgical treatment, washing the wound with antiseptic solutions with mandatory subsequent drying, so that it covers the surface with a uniform layer (0.5-1 mm) wounds. With extensive lesions, for example, with scalped wounds, a single use of 8-12 g of the drug is possible. After applying Kvadro, the wound is drained according to the general rules and a bandage is applied. With common wound tissue defects, loose packing with gauze napkins is allowed. The remnants of the Quadro preparation, together with the discharge from the wound and the bandage, are removed, the wound is dried with gauze pad and the preparation is additionally applied in a single dose without any other manipulations in the wound.
For the treatment of burns, the drug is applied to the burn surface both in the first 5-6 days and during the sequestration of the burn scab (up to 15 days). In this case, the drug is fixed on the wound using an aseptic bandage. During dressings, the drug should be completely removed from the surface of burn wounds using antiseptics or special shampoos for wounds. The use of the drug is possible on any part of the animal’s body in case of burns.
In cases where the surgical treatment of the wound cannot be performed in full, the Kvadro preparation is used to fill the wound with a dose of 0.5-4 g, depending on the area of the lesion, and apply a bandage for a time not exceeding 24 hours.
Contraindications
Hypersensitivity to gentamicin sulfate and other components that make up the drug.

When using the drug, adverse reactions may occur in the form of erythema, urticaria, contact dermatitis, itching.
Special precautions for use
The drug Kvadro can be applied to the surface of wounds only after thorough surgical treatment. The drug prevents the dressings from sticking and causing pain when they are removed. The drug does not have a local irritant effect, however, do not allow the drug to get into the eyes.
Use during pregnancy and lactation
Due to the lack of information on the absorption of gentamicin with external application, before using the drug in pregnant and lactating animals, the benefit / risk ratio should be carefully weighed.
