Description
Suspension from light yellow to yellow.
Composition
1 ml of the drug contains active ingredients: neomycin sulfate – 5000 IU, colistin sulfate (polymyxin E) – 10000 IU, lidocaine hydrochloride – 40 mg, monosulfiram – 50 mg, dexamethasone sodium phosphate – 1 mg.
Excipients: polysorbate 80 (tween-80), dimethyl sulfoxide, propylene glycol – up to 1 ml.

Otohels refers to complex preparations for local use, has antimicrobial, anti-inflammatory, anti-allergic and local anesthetic effects.
Neomycin, which is part of the drug, is a broad-spectrum antibiotic from the aminoglycoside group. The mechanism of antibacterial action occurs due to the active suppression of the synthesis of proteins of the bacterial cell. Neomycin has bactericidal activity against many gram-positive and gram-negative bacteria. One of the most active antibiotics against staphylococci, which often cause otitis externa, it also exhibits a fairly high activity against Proteus spp. Strains of streptococci and Arcanobacteria pyogenes are less susceptible. Cross-resistance exists with minoglycoside antibiotics.
Colistin (Polymyxin E) is a polypeptide antibiotic that is primarily active against gram-negative bacteria. It destroys bacterial cell membranes. It is one of the most active antibiotics against bacterial species that are often found in ear infections in dogs, cats and decorative rabbits such as: P. aeruginosa, Acinetobacter spp., E. coli, Enterobacter spp., Salmonella spp., Shigella spp. ., Klebsiella spp., Haemophilus influenza, Bordetella pertussis and Legionella pneumophila. Colistin is also active against Proteus spp., But some resistant strains are available. Chromosomal resistance rarely occurs. There is cross-resistance with other polymyxins.
Dexamethasone, a fluorinated glucocorticoid with an anti-inflammatory effect, is 25 times stronger than hydrocortisone. It also has antipruritic and anti-allergic effects.
Monosulfiram is an organic sulfur derivative with antiparasitic activity against the ear mite Otodectis cynotis and fungicidal activity against the yeast Malassezia pachydermatis (Pityrosporum canis) and Sandida albicans.
Lidocaine hydrochloride is an effective local anesthetic, which is used to carry out anesthesia of various types (terminal, infiltration, conduction). Lidocaine hydrochloride stabilizes cell membranes, blocks sodium channels, increases membrane permeability for potassium ions without disturbing conductivity, and accelerates the process of repolarization of cell membranes. Lidocaine hydrochloride is a local anesthetic that provides rapid pain relief when applied topically, is well tolerated by patients, does not affect respiration and blood pressure, and does not have a local irritating effect.
When applied topically, high concentrations of active ingredients reach the ear canal, where they show their effect.
Application
Otohels is intended for the treatment of small unproductive animals (dogs, cats and decorative rabbits) with otitis media of various etiologies, including those caused by ectoparasites and / or complicated by fungal or bacterial infections, pathogens of which are sensitive to neomycin sulfate and colistin sulfate (polymyxin E), and also with eczematous and allergic otitis media or ulcerative changes in the external auditory canal.
Dosage
Shake the contents of the bottle thoroughly before use. Otohels is instilled in each ear once or twice a day. The ear canals are cleaned of earwax, scabs and crusts. The drug is injected into each ear in doses:
- dogs weighing up to 5 kg, cats and rabbits of decorative breeds – 3 drops;
- dogs weighing from 5 to 10 kg – 5 drops;
- dogs weighing more than 10 kg – 10 drops.
For the purpose of a more complete treatment of the entire surface of the auricle and ear canal and deeper penetration of the drug, the auricle is folded along and slightly massaged at its base.
In acute otitis media, the course of treatment is 7-10 days. In the chronic course of the disease, the duration of treatment is determined by the veterinarian.
If one or more doses are missed, the drug should be restored in the same dosage and according to the same scheme.
Contraindications
Hypersensitivity of the animal to the components of the drug. Otohels should not be used in animals with an affected tympanic membrane.
Overdose
Overdose symptoms of the veterinary drug were not identified.
Side effects
When using the drug in accordance with this instruction, side effects and complications, as a rule, are not observed. After instillation of Otohels, erythema or itching may occur, which quickly pass. Desquamation may appear in the ear canal, which does not interfere with the healing process.
Warning
The drug should not be used in conjunction with other topical medicinal products.
Personal prevention measures
When working with Otohels, you should follow the general rules of personal hygiene and safety measures provided for when working with veterinary drugs. When working with the drug, do not smoke, drink or eat. After finishing work, hands should be washed with warm water and soap.
People with hypersensitivity to the components of the drug should avoid direct contact with Otohels. In case of accidental contact of the drug with the skin or mucous membranes of the eyes, they must be rinsed with plenty of water. In case of allergic reactions and / or in case of accidental ingestion of a veterinary drug into the human body, you should immediately contact a medical institution (you should have instructions for use or a label with you).
It is forbidden to use empty containers from the medicinal product for domestic purposes, it must be disposed of with household waste.

Storage

Shelf life is 2 years.
The shelf life after opening the bottle is no more than 28 days, provided that it is stored in a dark place at a temperature of 2 ° to 8 ° C.
